This site is not a patient portal

Please do not send protected health information, medical records, test results or any clinical detail about an identifiable person through the forms on this website. The inquiry form is a business contact channel. It is not designed, and should not be used, for clinical communication.

If you need to share clinical information with a healthcare service, use the channel that service provides for that purpose. If you believe you have a medical emergency, call your local emergency number.

Information we collect

We collect only what you choose to send us, plus basic technical data:

• Inquiry details. When you submit the contact form: first and last name, organization, email address, optional phone number, the area of interest you select, and the content of your message.
• Technical data. Standard server request information such as IP address, browser type and the pages requested, processed to deliver and secure the site.

We do not ask for, and you should not provide, government identification numbers, payment details or health information through this website.
